It's a good idea to have all the required information to hand before beginning the referral process.
First, we'll need to see evidence of how the user's Brain in Hand licence is being funded. For example, if it's funded through the Disabled Students Allowance or the Access to Work scheme, we'll need to see a letter that confirms this.
If the licence is being paid for by an organisation such as the user’s workplace or place of study, you’ll need to provide the details of the purchaser.
We’ll also need some basic information about the user including their:
- name
- address
- date of birth
- email address
- phone number
If the user will have a supporter to help them with Brain in Hand, we'll also need their contact details, along with any additional information that may affect the user’s suitability for Brain in Hand.
If the user is under 18, we'll need to contact their parent or guardian, so it's important to tell them our customer support team will be in touch.
